A licensed clinician will come to your home and conduct a thorough wellness evaluation (approximately 30 minutes). During the visit, your provider will listen to any health concerns, check your vital signs and review symptoms, review your medications, discuss your medical history, and provide you with education and a follow-up list for your primary care doctor. You'll receive a written summary of your visit.
